MANDATORY FORMS to be completed for each Camper

The forms listed below must be completed and submitted for each camper prior to the start of camp. 

As soon as possible, please submit the forms before camp starts (scan/photo and email to [email protected], mail to MYC, 1 Youth Way, Media, PA 19063, or drop off to MYC to the secure lock box located outside the office entrance of the building).
*Campers are not permitted to attend unless all required forms are on file!*

If any camper's healthcare provider, educational provider, social service provider, or other caregiver needs to visit MYC during the camp day, you (the registering parent/guardian - not the provider) must provide MYCamp with at least 2 business days' notice of each requested visit and the requested date, time, and duration of the visit.  MYCamp will try to reasonably accommodate all requested provider visits.  For the safety of our campers and staff, MYCamp will not permit any provider access to  MYCamp or any campers unless and until such provider has delivered to MYCamp copies of all required clearances, credentials, and photo ID.  Please email [email protected] to obtain information on the required MYCamp clearances and credentials, and to provide the advance notice of the requested visit.
